Permission
Letters
The following are sample model letters requesting
permission to use copyrighted works. These model letters should
be reviewed and modified to fit the specifics of your request. Your
letter should provide complete and accurate information and include
a self-addressed stamped envelope to expedite the permission process.

your permission letter, see Appendix 2.
Obtaining Permission from the Copyright Owner. Many publishers
also have online copyright permission pages that can be used for
this purpose. For more information on locating copyright holders
